How to Prescribe Nerivio® - A Quick Guide for Healthcare Providers

Nerivio is covered by many major U.S. commercial health plans, including Cigna, most Blue Cross Blue Shield plans, including Anthem/Elevance, BCBS Federal Employee Plan, Highmark, CareFirst, IBX, Premera, Horizon, and many more. Coverage continues expanding rapidly.

To ensure smooth fulfillment, fast patient access, and full claims filing and reimbursement handling by the NerivioNow dispensing partner, please follow the steps below.

Choose Your Prescribing Method

OPTION 1:

ePrescribe (EMR) to NerivioNow

OPTION 2:

Fax (or eFax) the NerivioNow Enrollment

STEP 1:

In your EMR, search for “Nerivio” and select the device. Note that this varies across different EMR platforms, and you may find Nerivio in the device listing and not necessarily in the drug listing.

STEP 2:

Select the retail pharmacy:

STEP 3:

In the Notes section, include the name “Nerivio”.

STEP 4:

Dispense Quantity and Number of Refills:

  • If you prescribe for prevention (or dual-use), it is recommended to indicate a 90-day supply (For annual prescription: Dispense Quantity - 3, Number of Refills - 9)

  • Alternative filling quantities may be selected based on your clinical judgment and the patient needs

STEP 5:

Choose ICD-10 diagnostic code (e.g., G43.XXX)

STEP 6:

Instructions for use (SIG):

  • Preventive treatment: 45-minute treatment every other day, regularly

  • Dual Use (preventive and acute treatment): 45-minute treatment every other day, plus upon onset of migraine attack

  • Acute treatment: 45-minute treatment at onset of every migraine attack

STEP 7:

Fill in clinical information

STEP 8:

IMPORTANT: Please fill in prior prescribed migraine medications tried/failed; this is extremely important, as it may save the need for prior authorization.

STEP 9:

Attach to the notes:

  • Patient demographics

  • Insurance details

STEP 1:

Download the prescription enrollment form below and fill out all the fields to prevent delays in claims processing.

STEP 2:

FAX the below to NerivioNow at 1(888)214-0258

  1. Filled prescription enrollment form

  2. Chart notes

  3. Additional clinical criteria (if applicable)

  4. Insurance details

IMPORTANT:

  • Please complete all the fields in the form to prevent delays in claims processing.

  • Missing information delays claims filing, prior authorization review (if required), and device delivery.

  • Providing all clinical and coverage details upfront reduces office burden and minimizes follow-up calls.

  1. Patient demographics & contact information

  2. Insurance information

  3. Prescriber NPI & contact information

  4. ICD-10 diagnostic code

  5. SIG (Acute / Prevention / Dual Use)

  6. Fill / refill quantity

  7. Tried/failed therapies

  8. Prescriber signature & date

  9. (Patient signature not required; verbal attestation accepted)

Instructions to Share with the Patient

Copy the below patient-ready instructions to email the patient after prescribing Nerivio.

Your prescription for the Nerivio device has been sent to NerivioNow.

The best course of action is to call them during normal business hours to set up the delivery of your device.

Download the Nerivio app and sign up for your Nerivio account. It is better to do that as soon as your healthcare provider appointment is done – before you even receive the device.

Directions for Use:

For Acute (Abortive) Treatment

  • Start treatment within 60 minutes of migraine attack onset

  • In the first few minutes, set the intensity to a strong but comfortable level, preferably higher than 20% (higher than 16% for children).
    Maintain the same intensity level for the full 45-minute treatment, but if sometime during the treatment the intensity feels weak, increase it until it feels strong but comfortable.

For Preventive (Prophylactic) Treatment

  • Perform one 45-minute treatment every other day at your convenient time, e.g., in the evening.

  • To remember your treatment days, choose either even or odd days of the week

  • If you treated acutely (abortively) earlier on a day of a preventive, you may skip the preventive treatment that night.

  • In the first few minutes, set the intensity to a strong but comfortable level, preferably higher than 20% (higher than 16% for children).
    Maintain the same intensity level for the full 45-minute treatment, but if sometime during the treatment the intensity feels weak, increase it until it feels strong but comfortable.

Each device includes 18 treatments, after which a refill is required. Do not wait for the last minute to request a refill – the Nerivio mobile App shows you how many treatments are left; once you’re down to 6, click on the REFILL button, or call the company which sent you the device.

Nerivio may be used more than once per day if needed. Nerivio can be used together with migraine medications.
